Community Profile(s) due October 31

Phase 1 of the Tobacco-Focused Community Assessment comes to a close at the end of this month when local public health agency grantees submit their Community Profile(s) to their STEPP project officer by October 31. Remember to either make a copy of the profile template that was provided as a Google doc in the Phase 1 Guidance document or download the template to use it. Phase 2 will run from November to December and includes assessing community readiness and agency/leadership readiness, selecting your strategy(ies), and developing your implementation plan.
