Evaluation Templates for Grantees

The five evaluation documents below will help you develop and submit your evaluation plans. Grantees are not required to use these templates, however, these documents are consistent with the types of information/questions you are asked when using the evaluation online module. STEPP encourages grantees to use these: 1. The Good Evaluation Questions Checklist provides guidance to help determine good evaluation questions. 2. The Evaluation Plan Outline (template) provides an outline of what should be included in the evaluation plan narrative. It contains a few tables to be completed and offers some direction to help grantees plan. This outline is a modified version of the of the Evaluation Plan Full Template (below). 3. The Evaluation Plan FULL TEMPLATE document offers in depth detail on each section of evaluation planning. We included this as a guide to help grantees who want more information on specific sections of the outline or if they simply want to seek greater understanding of evaluation planning. 4.
