- UPDATED: Community Profile Guidance Document
The guidance document above has been updated to include clarifying guidance for grantees funded at $38,000 or less (our minimally funded grantees). You will find all clarifications highlighted.
- Profile Template
A tobacco-focused community profile is required. The profile template that STEPP created is optional for grantees to use. You may use a different format or create a more detailed profile document. We recommend a 2-4 page Executive Summary with links to the data tables and results of key informant interviews from the guidance document. For profile instructions, please refer back to the Tobacco-Focused Community Profile Guidance Document. See page 7 for an overview of step 2, “Interpreting the data and creating a profile”. Further guidance on creating your profile can be found on page 24.

Secondary Data
STEPP is pleased to share county, Health Statistics Region (HSR) and state level secondary data. You can view the data
which includes BRFSS, CHS, HKCS, PRAMS and Census/ACS data. You are able to select and download your specific data from the main page of the dashboard. In addition to the data tables, county and HSR maps of tobacco/social determinant indicators are now available. To see descriptions of secondary data, key tobacco-related resources, data resources for demographic and determinants of health data, as well as links to additional resources and fact sheets, refer to these appendix tables.
